How to Evaluate an Online TRT Clinic Without Getting Burned
Testosterone replacement therapy is a legitimate, evidence-based treatment for men with confirmed hypogonadism. It is also a category that attracts a lot of marketing, some of it aimed at men who do not actually need it. The notes below are how we think about distinguishing serious online TRT care from glorified anti-aging marketing — and why the price-leader option is rarely the one we recommend.
Diagnosis Comes Before Prescription
Endocrine Society guidelines define hypogonadism as the combination of consistent symptoms (low libido, fatigue, mood changes, reduced muscle mass) plus two early-morning serum total testosterone measurements below approximately 300 ng/dL. Both criteria matter. Symptoms without low labs do not warrant TRT — they warrant looking for other causes. Low labs without symptoms also do not warrant TRT in most clinical frameworks.
A serious online TRT clinic will require lab work before the first prescription, will require two measurements before starting therapy (because testosterone fluctuates significantly day-to-day), and will require AM-window collection (testosterone is highest in the morning). Clinics that skip any of these steps are taking shortcuts that a clinical endocrinologist would not. We have seen telehealth platforms prescribe testosterone after a single afternoon-collected lab — that is a yellow flag, not a feature.
What Good Clinics Monitor While You're On Therapy
TRT is not 'set it and forget it.' The medications and clinical patterns that need ongoing follow-up include: hematocrit (testosterone can elevate red blood cell counts and increase clot risk), estradiol (some testosterone is aromatized to estrogen, and high estradiol contributes to gynecomastia and water retention), PSA (especially after age 40, to monitor for prostate changes), and serum testosterone trough/peak (to confirm dosing is in the appropriate range — typically 500-900 ng/dL on therapy).
A clinic that prescribes testosterone but does not order follow-up labs at 8-12 weeks and again at 6 months is not practicing TRT to current standards. The clinics in our top tier all build that follow-up cadence into the price; the lower-tier clinics tend to charge per-lab or skip them entirely.

Fertility Preservation Is a Real Conversation
Standard testosterone therapy suppresses the body's own LH/FSH signaling, which can shrink the testes and significantly impair sperm production. For men who plan to have children — including men who are not actively trying right now but might in the future — this is a conversation that needs to happen before starting TRT, not after.
The tools available include hCG (human chorionic gonadotropin, which mimics LH and preserves some endogenous testicular function), enclomiphene (a selective estrogen receptor modulator that increases LH/FSH and stimulates endogenous testosterone production), and pre-treatment sperm banking. Clinics that build hCG or enclomiphene protocols into their offering are operating at a higher clinical bar; clinics that do not even ask about fertility goals during onboarding are operating at a lower one.
The All-In Cost (Including Labs)

Bottom line: if a clinic will prescribe testosterone after one lab draw, no symptom screening, and no follow-up plan, what they're selling isn't TRT — it's testosterone access.
