What Is Online TRT and Who Qualifies?

Testosterone replacement therapy (TRT) treats hypogonadism — clinically low testosterone confirmed by blood testing. Normal total testosterone ranges from 300–1,000 ng/dL; most providers initiate treatment when levels fall below 300 ng/dL in the presence of symptoms such as fatigue, low libido, reduced muscle mass, and mood changes.

Online TRT platforms have made diagnosis and treatment significantly more accessible. The standard workflow is: order a home blood test (or visit a local lab), complete a telehealth consultation, receive a prescription, and have medication shipped directly. Most platforms can complete this process within 3–7 days.

Testosterone is a Schedule III controlled substance. Prescribing requires a valid clinician-patient relationship, a confirmed diagnosis, and ongoing monitoring. Reputable online TRT providers require baseline labs and periodic follow-up testing (typically every 3–6 months) to monitor hematocrit, PSA, and testosterone levels.

Delivery Routes: Injections, Creams, Troches, and Oral

Testosterone cypionate and enanthate injections are the most cost-effective delivery route, typically $30–$80/month for medication alone. They produce stable serum levels when injected weekly or twice-weekly. Most online TRT providers offer injectable testosterone.

Topical creams and gels (AndroGel, Testim, compounded creams) are applied daily and avoid the injection barrier. They cost more than injectables ($100–$250/month) and carry a transfer risk to partners and children if skin contact occurs before the application site dries. Compounded creams are widely available through online TRT platforms.

Troches (sublingual/buccal lozenges) and oral testosterone undecanoate (Jatenzo, Tlando) are less common but available through some platforms.
