§ Editorial Overview

Online Hair Loss Treatment: Finasteride, Dutasteride, and Minoxidil

Androgenetic alopecia (male and female pattern hair loss) is driven by dihydrotestosterone (DHT) binding to hair follicle receptors, causing miniaturisation. The two primary pharmacological interventions are 5-alpha reductase inhibitors (finasteride, dutasteride) which reduce DHT production, and minoxidil which prolongs the anagen (growth) phase of the hair cycle.

Finasteride 1 mg daily is FDA-approved for male pattern hair loss and has been the standard of care since 1997. It reduces scalp DHT by approximately 60% and has demonstrated hair regrowth or stabilisation in 83–90% of men in clinical trials. Dutasteride 0.5 mg daily inhibits both 5-alpha reductase type 1 and type 2 (vs finasteride's type 2 only), producing greater DHT suppression (~90%) and potentially superior efficacy, though it is only FDA-approved for BPH.

Topical minoxidil (2% for women, 5% for men) is available over-the-counter. Oral minoxidil (0.625–5 mg daily) is increasingly prescribed off-label by telehealth platforms and shows superior efficacy to topical in some studies. Compounded multi-active topicals combining finasteride, minoxidil, and other actives (tretinoin, biotin, caffeine) are a growing category.

Pricing for Online Hair Loss Treatment

Generic finasteride is one of the cheapest prescription drugs available — $10–$30/month from most online platforms. The telehealth consultation and platform subscription add $10–$50/month, making all-in costs $20–$80/month for a basic finasteride regimen. Compounded topicals and combination formulations cost $60–$200/month.

Platforms that specialise in hair loss (Hims, Ro, Strut Health) offer the widest formulary including compounded topicals, oral minoxidil, and combination products. General telehealth platforms (PlushCare, Sesame Care) can prescribe finasteride but typically do not offer compounded formulations.

Hair Loss: What the Studies Show

Evidence rows reference published clinical trials. Finasteride and dutasteride are contraindicated in women of childbearing potential. Oral minoxidil requires blood pressure monitoring.

EndpointFinding
Finasteride 1 mg (2-year RCT)183% of men showed no further hair loss; 66% showed measurable regrowth vs 7% placebo
Dutasteride vs finasteride (12 months)2Dutasteride 0.5 mg showed significantly greater hair count increase vs finasteride 1 mg in Korean RCT
Oral minoxidil (6 months)3Oral minoxidil 5 mg significantly superior to topical minoxidil 5% for hair density in RCT
Combination therapy1Finasteride + minoxidil combination shows additive benefit over either agent alone in observational studies

Combination therapy is the standard approach for moderate-to-severe androgenetic alopecia.
