How Much Does Compounded Tirzepatide Cost per Month? Prices by Dose

Compounded tirzepatide is advertised from under $100 a month, but that number usually describes the first month or the lowest dose. Here is what providers actually charge at each step, what it costs once you reach a maintenance dose, and how a full year compares with brand-name Zepbound.

The Short Answer

At our October 21, 2026 check, compounded tirzepatide from telehealth providers cost roughly $86 to $300 a month once you reach a maintenance dose, all-in. The lowest prices came from providers that charge one flat price at every dose (Collective, WellMedr, Trimi). Providers that price by dose start cheaper and rise as the dose increases. For comparison, brand-name Zepbound bought directly from Eli Lilly costs $449 a month at maintenance doses, as long as you refill within 45 days.

Why the Price Depends on Your Dose

Tirzepatide is started at a low dose and increased over several months to limit side effects. Brand-name Zepbound starts at 2.5 mg a week and can go up to 15 mg. Compounded programs follow a similar ramp. Because higher doses use more drug, many providers charge more as you go up. Others charge one flat price at every dose, which costs more than a per-dose plan in the first months but is often cheaper once you reach maintenance.

Example: A Per-Dose Price Ladder

Mochi Health is one of the few providers that publishes a price for every compounded tirzepatide dose. Its ladder shows how per-dose pricing works in practice (membership not included):

Weekly doseMonthly medication price
2.2 mg$90
4.4 mg$119
6.6 mg$144
8.8 mg$174
11.24 mg$184
13.2 mg$194
16.6 mg$199

What a First Year Costs

The fairest comparison is a full year, because introductory prices and dose increases even out. The table below assumes one month at each step until a maintenance dose, then that dose for the rest of the year. Your own schedule depends on your clinician and how you tolerate each dose.

OptionHow we calculated itApproximate first-year cost
Collective (compounded)$69 × 12 + $199 membership$1,027
WellMedr (compounded)12-month prepaid plan$1,068
Trimi (compounded)$125 × 12$1,500
Mochi Health (compounded)$90 + $119 + $144 + $174 + $184, then $194 × 7, + $199 membership$2,268
Shed (compounded)$239 × 12 at the lowest advertised rate$2,868
Zepbound via LillyDirect (brand)$299 (2.5 mg) + $399 (5 mg) + $449 × 10, refilled on time$5,188

Costs That Don't Show Up in the Headline Price

  • Memberships billed separately from medication (common with brand-name programs, and some compounded ones such as Collective and Mochi).
  • Introductory prices that apply to the first month only.
  • Prepayment: the lowest monthly rates often require paying 6 or 12 months up front, and some providers bill every 12 weeks.
  • Dose surcharges: per-dose pricing rises as you titrate.
  • Cancellation terms: some multi-month plans require you to pay the remaining balance if you stop early.

How to Choose

  1. Ask for the price at your expected maintenance dose, not just the starting dose.
  2. Add any membership and check whether medication, visits and shipping are included.
  3. Ask which pharmacy fills the prescription and whether it is a state-licensed 503A pharmacy or an FDA-registered 503B outsourcing facility.
  4. Read the cancellation and refund terms before prepaying.
  5. Compare the total with brand-name options, especially if insurance might cover Zepbound.
